Essential Responsibilities:

  • Provides direct nursing service and care to patients in their residences as delegated by a home health registered nurse (RN) and/or physical/speech/occupational therapist (PT/ST/OT) and by the plan of care, thereby preventing hospitalizations and reducing or preventing inappropriate ER visits. Assists with or provides personal care to the patient safely and appropriately, as directed by the registered nurse or physical, speech, or occupational therapist. Helps patients adhere to their prescribed therapeutic regimen under the supervision of a registered nurse and/or a physical therapist, speech therapist, or occupational therapist. As assigned, renders basic domestic chore services that are indispensable for healthcare at home. Comprehends and implements universal infection control protocols. Maintains a safe, sanitary, and healthy environment for the patients under his/her care. He or she adheres to the aseptic technique when preparing apparatus and materials for treatments.
  • Documents and observes the status of the patient and the care or service provided. Comprehends the components of body function and the changes in body function that must be reported to the RN or PT/ST/OT and reports them promptly (licensed personnel may report directly to the patient’s physician). Develops clinical progress notes that assess and evaluate patient outcomes by employing descriptive, objective, and quantifiable data. Demonstrates proficiency in emergency procedures and maintains CPR certification. Recognizes and responds to emergencies by instructions until assistance is rendered.
  • Provides patients and caregivers with information regarding suitable self-care strategies. Engages in conferences with multidisciplinary teams. Participates in in-service training and continuing education programs to maintain awareness of patient care techniques and requirements. Assists social workers and nurses in the disposition of cases and provides support for the professionally developed plan of care. 
  • Upholds productivity standards established by the home health manager/director. Engages in activities that enhance performance. Assists in the formulation and execution of the department’s work plan. Conducts an audit of clinical records.
  • They may provide patient care to the extent required to ensure that the clinical expertise, competency, and licensing required to fulfill job responsibilities and direct the provision of care on the unit are maintained.
  • On an as-needed basis, administers direct patient care. Provides services that are in accordance with all legal, regulatory, and policy requirements pertinent to the clinical role performed and are within the scope of the license.

Basic Qualifications:

Experience:

  • A minimum of one year of experience in community health and geriatric nursing.
  • Within the past twenty-four (24) months, I have been employed in a community health service that serves the elderly population.

Education:

  • A bachelor’s degree in a healthcare discipline with certification as a home care aide or nurse aide, or completion of an approved practical nursing course.

License, Certification, Registration:

  • Licensed Practical Nurse (Hawaii)
  • Driver’s license (in the location where it is applicable)
  • Basic Life Support from the American Heart Association

  • Experience in a specialized nursing care facility or acute care facility.
