Description:

Duties and Responsibilities:
- Ensure that the medical health concerns of individuals are addressed. This may involve the following: scheduling appointments with the doctor, providing transportation to the appointment, adhering to the doctor’s orders, and communicating all relevant health issues to the supervisor. Monitors, observes, and encourages individuals to self-administer medication.
- Incorporate hands-on hygiene prompting and fulfillment into daily activities to provide assistance to individuals.
- Assist in the development of independent living skills and vocational skills, which may involve employment applications, interviews, job coaching, or other skill development.
- Executes behavioral objectives, instructional strategies, monitoring protocols, exercise regimens, and any other programs and procedures that the team has recommended.
- Assist in the completion of individual assessments as required.
- Assist and supervise the preparation of food for individuals who adhere to specific prescribed diets and food consistency. Eating procedures are implemented in accordance with their written form, when applicable.
- Offer individuals meaningful community experiences.
- Ensure that the level of supervision of individuals in the community and at home is maintained in accordance with the individual’s plan and critical information.
- As directed, document case notes, communication records, programs, incident reports, abuse and neglect reports, safety reports, fire drills, timecard exception sheets, mileage forms, and other necessary documentation in a timely and accurate manner.
- Role-model positive adult behaviors in the community and cultivate a sense of community through activities that are both individual-centered and appropriate.
Requirements:
Professional Qualifications:
- Comprehending the fundamental interactions between human systems, including the individual, group, organization, community, and society, and their implications for individuals with developmental disabilities.
- Proficiency in recognizing and selecting natural opportunities that foster the development and achievement of objectives for individuals with developmental disabilities.
- Proficiency in the execution of interventions for individuals with developmental disabilities.
- Process skills that are necessary for the implementation of services for individuals with developmental disabilities.
Education and/or Experience:
High school diploma or general education degree (GED); 3 to 6 months of related experience and/or training, or human service training certificate.Proficient in the utilization of fundamental computer programs.
Certificates, Licenses, Registrations:
- A Connecticut driver’s license that is valid
- Capacity to acquire the following licenses and certifications:
- Certification in CPR and First Aid, as mandated by the program site.
- PMT certification.
