This is a distinctive and thrilling opportunity to join the Forensic Intellectual and Neurodevelopmental Disabilities (FIND) Community Team in South London and Maudsley (SLaM). The FIND Team will provide specialist assessment, treatment, and consultation to the forensic Service to ensure a safe transition for inpatients discharged from the inpatient Forensic Services to the community.

This will ensure a clinically justified, smooth, and secure pathway for individuals with mental illness, offending history, and learning disability, whether or not they have a neurodevelopmental disorder. South London Mental Health and Community Partnership (SLP) is a partnership that is comprised of the three forensic agencies of SLaM, South West London and St Georges (SWLSTG), and Oxleas. The SLP’s overall objectives are to enhance patient pathways and outcomes by providing exceptional care in superior environments and bringing care closer to home.

The SLP has a history of delivering service developments that are clinically informed and highly effective, thereby better meeting the requirements of service users. The SLaM FIND Team will collaborate closely with the FIND Teams in each of the other trusts to facilitate the transition of service users from inpatient settings to community provision. The FIND provision is an example of such a development.

The incumbent will offer a highly specialized service to a predetermined caseload of service users and their caregivers, which will encompass social supervision and care coordination. A team of multiprofessionals with specialized talents and experience in the client group is the MDT team.

The FIND Team will receive a specialized social work service from the post holder. The primary responsibilities consist of:

  • To assume a leadership role in the delivery and coordination of a social work service to service users and their families/carers within the FIND Team.

  • Acting as a point of contact to attend CPAs, assertively manage discharge pathways, and reduce the risk of delayed discharge, the SLP partnership is to maintain a clear understanding of the patients who are placed in secure and specialist services.
  • To oversee a limited caseload, which typically includes individuals with complex mental health and social requirements. This will encompass the following: the planning of care, the implementation or supervision of treatment, the review of care, the communication with other professionals and agencies involved in the care, and the attendance of pertinent reviews and meetings as necessary.
  • To engage in the monitoring and evaluation of patients who are in medium and low-security conditions both within the SLP and outside of the area. The individual in this position is expected to establish personalized and timely pathways in the least restrictive care/support settings, in accordance with the individual’s recovery objectives, while also capitalizing on their strengths.
  • The individual in this position must be capable of conducting comprehensive social care needs assessments and collaborating closely with other health providers, placement providers, and other critical stakeholders.
  • To facilitate and enhance step-down pathways by providing personalized care packages that capitalize on individuals’ strengths and ensure timely discharges.
  • To negotiate between various directorates, liaise with local health care and commissioning teams, and work with the community care coordinators to facilitate the transfer from out-of-borough service provision.
