A Certified Homemaker-Home Health Aide (CHHA) is an employee of the company who is responsible for ensuring the client’s safety, dignity, well-being, and ability to continue residing at home. The CHHA visits the client’s residence to administer direct care by a written Plan of Care that encompasses personal care, hygiene, ambulation, special procedures, homemaking, meal preparation, housekeeping, and assistance with other activities of daily living.

The CHHA is an employee who is under the supervision of a registered nurse. This position does not involve any supervision obligations. The CHHA is a Direct Care staff member who is hourly per diem and non-exempt and has HIPAA-restricted access to specific client information. There is no guaranteed minimum number of hours per week.

Additionally, clients are instructed to engage in prescribed exercises. Assist the client in the use of assistive devices, such as mechanical lifts, walkers, wheelchairs, commode chairs, braces, and prostheses, as well as for ambulation and transfer activities. Execute special delegated procedures, such as assisting the client with self-administered medications, feeding, measuring intake and output, and recording vital signs and weight. These activities necessitate consistent attendance at scheduled assignments and the fulfillment of a diverse array of physical requirements, such as those delineated in the Working Conditions and Essential Functions section below.
  • Perform minor housekeeping, meal preparation, and other support services at the client’s residence as part of the care plan. This encompasses a variety of responsibilities, including the following: menu planning and shopping lists, errands, meal preparation (including special diets), presentation of food, and subsequent cleaning of the work area, appliances, and dishes. Additionally, it includes the following: grocery shopping, dusting, laundry, vacuuming, and the general cleaning of the bathroom, kitchen, and living area when specified in the written plan of care. The performance of these activities necessitates consistent attendance at scheduled assignments and the use of a diverse array of household equipment and appliances, as well as the physical demands outlined in the Working Conditions and Essential Functions of this Job Description.
  • When the client is in the home, it is important to monitor their condition, behavior, appearance, hygiene requirements, living arrangements, and home environment. Any changes or issues should be reported and documented to the appropriate staff member.
  • Write or otherwise electronically document visit reports to accurately record the care provided in the home, electronically record time and attendance, and complete other forms to document the work of this position, including incident reports and written time and attendance reports as instructed.   • Be punctual for scheduled visits, be consistently available for assignments, and maintain consistent attendance. Contact the office frequently for assignments or when you are tardy with an assignment.

This role necessitates the following: the ability to drive a car or use public transportation independently, the ability to lift, bend, reach, kneel, push and pull, stretch, stand, stoop, walk, walk up and down stairs, see, hear, speak, write, read, carry, engage in weight-bearing activities, and the use of a diverse range of home appliances, both large and small.
